电灼去除处女膜伞的临床应用价值探讨

   ［摘要］  目的  探讨电灼去除处女膜伞的临床实用价值。方法  对2000例处女膜伞的患者电灼后的结果进行分析。结果  1978例1次电灼成功，治愈率98.9%。3例因去除不完全而第2次电灼解除症状，19例失败，进一步检查证实为膀胱出口梗阻，膀胱逼尿肌-尿道内括约肌协同功能失调，做了相应处理。结论  处女膜伞是引起排尿功能障碍的疾病之一，应用电灼法治疗，操作简单易行，安全有效，宜于推广。对于合并有其他因素共同引起排尿功能障碍的患者，除做电灼切除处女膜伞之外，亦应同时治疗其他合并症。

　　Study on clinical value of electrocauterization therapy in treatment of hymen umbrella

　　［Abstract］  Objective  To evaluate the clinical value of electrocauterization therapy in the treatment of hymen umbrella.Methods  Analyzed the treatment results of 2000 cases of patients with hymen umbrella after electrocauterization therapy.Results  1978 cases are successful only by one electrocauterization therapy,cure rate is 98.9%.Because of being cut off incompletely,8 cases are relieved the symptoms after receive the treatment of electrocauterization again.14 cases are validated respectively as bladder neck obstruction or bladder detrusor-urethral internal sphincter dyssynergia and treated accordingly.Conclusion  The hymen umbrella is one of the diseases to cause the urethral dysfunction.The electrocauterization therapy is simple,convienient,safe and effective,it can be spreaded easily.To patient who merge other factors,it is necessary to treat other syndromes at the same time in addition to cauterizing away the hymen umbrella.

　　处女膜伞是易被误诊的疾病之一，它属于尿道综合征的一种。自1996年6月～2003年1月，我院用电灼法治疗2000例处女膜伞的患者，治愈1978例，现报告如下。

　　1  资料与方法

　　1.1  一般资料  本组2000例女性，年龄20～83岁，平均46.5岁。其中1例20岁，未婚，但有性生活史；其余皆已婚。就诊原因：尿频，2个月～18年不等，曾在多家大、中型按泌尿系感染反复治疗，未愈；或被拟诊为膀胱炎、尿道炎、精神性尿频、肾盂肾炎、肾结核等病。尿常规检查几乎全部正常；多次中段尿培养为阴性；外阴检查：尿道外口被赘生物部分或全部遮盖。做IVP检查均无泌尿生殖系畸形。

　　1.2  治疗方法  2000例病人，经明确诊断为处女膜伞病后，均行电灼治疗。术前1%地卡因做局部黏膜表面麻醉，电灼去除尿道外口遮盖物，使尿道完全显露，电灼后自尿道置入F16号Foleys管至膀胱，气囊内注入水10 ml，尿管保留至次日晨取出。
