高血压心肌纤维化及其诊断与治疗进展

        4 小结与展望
         高血压病可导致心肌纤维化,其病因病理主要有高血压病理状态下的RAAS紊乱,多种细胞因子的异常表达,导致促纤维化信号通路的激活,心肌胶原的产生/降解失衡等,是多因素导致的疾病。心肌纤维化的诊断手段主要有生化、超声、MRI及心肌活检。ACEI、ARB、醛固酮拮抗剂、β受体阻滞剂、他汀类药物以及中医中药被证实可阻滞或逆转纤维化。但目前在高血压心肌纤维化的研究也存在一些问题,主要有实验模型的建立缺乏有效性与统一标准;临床诊断手段各有优缺点,很多尚处于探索阶段;在中医研究领域,缺少对高血压心肌纤维化的中医基础病理的理论支持及中医整体证候分析与分型,而往往只注重微观研究;心肌纤维化的中医模型缺乏,缺少模型其与中医证候的相关性研究等。综上所述,抗心肌纤维化的药物研究临床应用广泛,值得进一步探讨及研究。
